Use teleconsent to get a patient’s consent before or during telehealth appointments. (Learn how to use teleconsent.)

Available Forms

Standard teleconsent

This form may be used to obtain consent from a patient to be seen via telehealth.

Release of Information

This form may be used to obtain consent from a patient to discuss their personal health information with another party.

Credit Card Authorization

This form may be used to obtain authorization from a patient to charge their credit card.

Acknowledgement of Privacy Practices

This form may be used to obtain acknowledgement from a patient that they have received a copy of your office's privacy practices.

Consent to a Specified Document

This form may be used to obtain documentation that a patient has seen and consented to any specific document.

If the patient has not yet seen the specified document, you could use doxy.me's Screenshare feature to review the document with them. Once you've done this, you can then open teleconsent and share this consent form to obtain the patient's signature.
