Send a Form
Sending a form from the Dokbot portal requires an account. (Dokbot: Signing Up for Dokbot)
How To Send A Form
Find the name of the form to be sent to your patients. You can search for your form from the search bar, as well. Then click on the form to be sent to the patient and click “Continue”.
Enter your information, specifically how the patients would recognize you when they receive an email then click on “Continue”.
Fill in your client’s name and email address to where you would like to send this assessment to then click on “Continue”.
Check the details and confirm to send the Dokbot form.
Great! You have send the form; you can opt to send the Dokbot form link personally or send them a e-mail on their assigned email.
